** *POSITION: Nursing Assistant NON-EXEMPT * *SUPERVISOR: Licensed Nurse (RN or LPN) PAY GRADE: B* *UNIT: Carlyle Care Center REVISION DATE: 8/3/12* ** *PRIMARY RESPONSIBILITIES:* ** To perform all clinical duties as instructed by licensed nurse. ** Responsible for accounting for all narcotics and routine medications throughout shift. ** Responsible for maintaining accurate resident records. * * * * *ESSENTIAL DUTIES: * ** Give a complete shift report at the beginning and end of each shift, including a walk through. ** Provide medication assistance for residents and complete required electronic charting ** Perform narc count during shift report and any time keys exchange hands, following med assist procedures. ** Update resident charting of care given and observations on a daily basis ** Assist residents with bathing on a daily basis, per posted bath schedules ** Take resident vital signs on a monthly basis, or as requested by licensed nurses ** Cue residents for meals ** Serve breakfast, lunch, and dinner trays and complete meal monitor. ** Assist residents with ADL’s on a daily basis. ** Cleaning of resident care equipment as needed and/or per posted schedule. ** Making and changing resident beds as needed. ** Assist residents with toileting and incontinence needs. ** Cleaning of spills, messes or bodily fluids that occur after housekeeping hours.** ** Follow posted break schedule. ** Cover front desk during posted break schedules. ** To display a pleasant, interested and positive approach to residents. ** To be an active listener to resident needs and concerns. ** To maintain and respect privacy and confidentiality in relationship to both residents and staff. ** To treat all residents and staff with respect and dignity. ** To work in a team setting with staff from various departments. ** To attend all in-service education programs and staff meetings required of all personnel. ** To document all care given and all observations made while providing care. ** To report to licensed nurses immediately when observing any unusual or significant change(s) in a resident’s physical or behavioral condition. ** To seek out assistance when unsure of assignment, procedure, or situation. ** Adhere to company policies, procedures, and management decisions. ** Ensure that proper hand washing, hygiene, and infection control techniques are exercised. ** Other duties as assigned. * * * * *PHYSICAL REQUIREMENTS:* ** This position requires standing and walking for two hours at a time, up to eight hours out of an eight-hour day. ** Lifting ranges up to 10 pounds frequently with seldom lifting of 25 to 50 pounds. ** There is occasional bending, twisting or kneeling performed, with occasional reaching and frequent handling and grasping. ** This position requires the ability to provide emergency resident care in whatever position the resident is found. ** This position requires constant sight and communication. ** Seldom use of Hoyer lift and Gait belt.** * * *MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:* ** High School Diploma or GED required. ** Must have Washington State Certified Nursing Assistant license or Home Care Aide license in good standing; or must meet DSHS exemption requirements by working as a long-term care worker in 2011 and having all required trainings in effect on 1/6/12; or must have been hired before 1/7/12 and have completed all required trainings within 120-days of hire. Exempt workers that are not licensed must obtain licensure within 90-days of employment. ** Applicant must be able to successfully pass a Washington State DSHS and FBI fingerprint background check. ** Must be able to navigate computer for electronic charting and e-mail communication. ** Basic computer skills including typing to produce accurate resident records. ** Preference given to applicants with mental health and medication assistance experience. ** Applicant must complete DSHS approved Safety and Orientation Training prior to working in the facility. ** Applicant must obtain a TB test within 72 hours of hire and must obtain a 2nd step test within allotted timeframe. If history of testing positive must provide proof of chest x-ray and treatment completed or blood test results indicating negative for TB prior to working in the facility. ** HIV/AIDS Training and CPR/First Aid Training must be completed within 30 days of hire unless proof of completion provided. ** Mental Health Specialty Training, Dementia Specialty Training, and Developmental Disability Specialty Training must be completed within 120 days of hire unless proof of completion provided.
